Cancellation, No-Show, Reschedule, Tardy: Appointment Policy Promos and Specials | June 1, 2023 Updated June 2023 Valued Patient – By scheduling an appointment with us, patients agree to our cancellation, no-show, reschedule and tardy policies. To schedule an appointment with us a $75 deposit is required in our MedSpa. This prepaid amount is non-refundable. If an appointment is rescheduled 48 hours before the appointment time, this prepaid amount will be credited for future services. Surgical Consultations must pay the consultation fee in full upon scheduling. NOTICE OF TARDY: Arriving more than 10 minutes late to any appointment will deprive one of their treatment time and the provider’s time. Arriving late to an appointment may result in cancellation and rescheduling, at our discretion, and a charge of our cancellation fee. If we are able to accommodate one in a safe and timely manner, we will make an effort to safely treat within the remaining time of the scheduled appointment. NOTICE OF CANCELATION OR RESCHEDULE: Our office follows a strict 48-hour cancellation and reschedule policy to ensure all patients can receive their treatments in a timely manner, while respecting everyone’s time. Any patient that reschedules/cancels within 48 hours of their appointment will automatically forfeit their appointment deposit. Fees will still be charged in the event that the appointment is scheduled by another party. Patients may be required to pre-pay for future services following a cancelation or reschedule within 48 hours of their scheduled appointment. Appointment deposits are non-refundable. NOTICE OF NO SHOW: New patients who no-show may be declined to be seen in the future. Established patients may be required to pre-pay for future services following a no show of an appointment.
